The Democratic Republic of Congo's (DRC) latest Ebola outbreak is a stark reminder of the complex interplay between armed conflict and infectious diseases, posing a significant threat to global health security. This outbreak, declared in May 2026, has already claimed 138 lives and infected 695 individuals, highlighting the urgent need for improved preparedness and response strategies.
One of the key challenges is the delayed detection of the Bundibugyovirus (BDBV), which causes Ebola virus disease. BDBV, with a reported fatality rate of 30-50%, has been detected in humans three times, including the current outbreak. The non-specific clinical presentation of BDBV and the limitations of diagnostic tools have led to undetected transmission events, a concerning trend that has been associated with larger and longer Ebola outbreaks in the past.
The outbreak also underscores the importance of understanding the One Health approach, which recognizes the interconnectedness of humans, animals, plants, and the environment. While fruit bats are believed to be the natural reservoir of BDBV, the precise mechanisms of zoonotic spillover events remain unclear. The rich bat biodiversity in Ituri Province, coupled with habitat loss and degradation, likely contribute to frequent spillover events. The porous nature of the DRC-Uganda border has facilitated the spread of the outbreak, with Uganda reporting 19 BDBV cases and two deaths as of June 2026.
Conflict and insecurity have further exacerbated the situation in the DRC. Since the 1994 Rwandan genocide, the country has been marked by ethnic tensions, political violence, and militant activities, particularly in Ituri, located along the DRC-Rwanda border. The presence of large reserves of rare-earth metals and minerals has attracted American and Chinese multinational mining companies, further fueling the conflict. The ongoing conflict has resulted in the displacement of approximately 5.6 million people within the DRC and severe food insecurity affecting an estimated 26.5 million people.

The US CDC models predict that the ongoing outbreak could reach the scale of the 2014-2016 West Africa outbreak. Until a safe and effective vaccine is approved, community-centric public health interventions must be prioritized. The unique challenges posed by the DRC, including severely weakened health infrastructure, armed conflict, and displacement, underscore the critical need for rapid, point-of-care diagnostics for BDBV and increased awareness of transmission-facilitating behaviors and practices.
